Exactly what Information is on a "DAC Report"?

I help drivers get their DAC report fast so they may correct errors, more easily complete employment applications, store it securely on-line and provide tools so the information can be used by drivers to get a job. Interestingly, one of the more common questions I get doesn’t deal with the above, but instead is more basic: “Exactly what’s on a DAC?”
When someone says “DAC Report” it can mean several different reports either stored or obtained by USIS (formerly called DAC Services). Most companies consider the “DAC Report” to be the employment history information stored at USIS by former employers. We’ll go over the categories to be found in that report later. First though, be aware that USIS also obtains (and reports) other information that may not be on your employment history “DAC Report.” (Articles on much of this information has been done in the past and the links will take you to past articles)This information includes:

The above information is obtained by USIS and outside of some criminal records, is not proprietary—other companies obtain the same information. The main type of information USIS has that others don’t is employment history information submitted by motor carriers…the “DAC Report.” (You may obtain your DAC report, MVR, Criminal Record/Infraction database scan and other information potential employers will see via computer at www.dotjobhistory.com.) You may also obtain your DAC report by mail by calling USIS at 800-381-0645)
The DAC Report employment history report contains the following categories of information:
- Identification of Driver. (Name, SSN, DOB)
- Identification of Contributing company (Address and phone number) and date contributed
- Period of Service (Month/year)
- License Number
- Eligibility for rehire
- Reason for leaving (e.g., driver quit or discharge, etc.)
- Status (e.g., owner operator, company driver, etc.)
- Driver Experience (e.g., single driver, team, etc.)
- Equipment Operated (e.g., reefer, dry box, etc)
- Loads Hauled (e.g., general commodity, etc)
- Work Record (e.g., satisfactory, cargo loss, etc.)
A separate section of the report contains accident/incident information. It is divided into DOT recordable and non-DOT recordable information. DOT recordable records contributed by employers will contain accident details as stored in the accident register. Non-DOT recordable accidents may contain details (date, short description, etc) if provided by the past employer.
It will also be indicated whether this particular company stored DOT drug and alcohol testing information with USIS. If the information is stored, the potential employer must send a written release to USIS to get the actual drug and alcohol results.
Time Periods
All of the above information may be found on a DAC employment history, but every report may not contain all the information. There are time periods beyond which, the information “drops off.”
- Three Years – Drug and alcohol information
- Seven Years – Categories of information that could contain derogatory information. Example: Section 11 as shown above will NOT be shown on any record over 7 years – even those records with “satisfactory” indicated.
- Ten years – The entire employment history record.
